Field types (example shown in images shown below):
Text Box | Single-line text box (for text, dates, numbers, etc.) Text Box questions can be validated to specifically hold dates, email addresses, phone numbers, MRN, numbers, etc. |
Notes Box | Large text box for paragraph style input of text |

Drop-down list | Lists all options in a drop-down box, need to click on box to view all options. Only one option can be selected – single choice |
Radio Buttons | Shows a list of all available options, has a round bubble next to them. Only one option can be selected – single choice |
Checkbox | Shows all options and has a square checkbox next to them. Allows more than one option to be selected – multiple selections |
Yes / No | Radio buttons with yes and no options; coded as 1, Yes | 0, No |

Descriptive | Text displayed with no data entry and optional image/file attachment. |
Signature | A signature field can be added to the project in which a participant can sign using the mouse or their finger. |
Begin New Section | A new section separator can be added to your instrument to aide in breaking out information. |
Matrix of Fields | A matrix allows you to display a group of similar multiple choice fields in a very compact area on a page. The key aspect of a matrix of fields is each choice is shared by every field (rows). In other words, each field in the matrix will have the same choices. You cannot have a field with unique choices in the matrix. |
